Child Registration Form

If your child will be attending any of the Fellowship of Champions’ youth ministries, please complete the registration form. To expedite registration, please do one of the following:

1. Complete the form, print and bring a signed copy with you.

   1. FOC Victory Zone/IGNITE Registration Form

2. Complete the Google Form and sign when you drop of your child(ren).

3. Arrive early to complete form(s) prior to leaving your child(ren) with FOC youth ministry staff.
